My Health Online/Fy Iechlyd Ar-lein offers you the convenience to book appointments online (when available). Depending on your practice you may be able to order repeat prescriptions online or change your contact details.

To protect patient confidentiality, you must provide documents so we can make sure you are who you say you are. When you next visit your practice please provide identification, preferably photo ID (e.g. driving licence).

During the current coronavirus outbreak, we are unable to verify identification in person so here is a suggestion on how you can provide documents:

• Download and print the registration form then complete. Post or deliver by hand in a sealed envelope to the practice letter box, together with a copy of photographic ID, for example driving licence or passport together with a copy of a recent utility bill (Within the last 3 months)
