Primary Care Pain Service

Chronic pain affects between one-third and one-half of the population of the UK. Consultations on pain account for 22% of all primary care attendances, at a time when demand for GP appointments is at an all-time high (CKS, 2021)

Our primary care-based pain management service supports patients to enjoy their full life despite the pain, by understanding their condition better, reducing their dependency on medication and encouragement to take back control of their life and condition management.

  • Patient-centred approach to de-medicalise and improve quality of life with self-management strategies.
  • Delivery of modern patient care, consistent with recent guidelines NICE (2021), AWMSG (2022)

 

What Can You Expect?

  • Assessment and discussion about the history of your condition.
  • Explanation of why it hurts, and what other factors are contributing to the level of pain.
  • Discussion about your medication to make sure you are on the most effective and safe treatment.
  • If during the conversation we decide that you will need the help of a different healthcare professional, we can refer you as appropriate.
  • Ongoing support, understanding, guidance and the ability to talk to someone about what you are going through.

 

Working in Partnership

  • Pain Practitioner – to explain pain, and its psychological links to your well-being, optimise your medication regime, and signpost to the most appropriate healthcare professionals to aid life improvement and recovery.
  • Physiotherapy – to restore function and reduce disability.
  • Social Prescribing – to signpost to a range of local services and facilities.
  • Mind, Silver Cloud – to provide mental health support.
  • Pain Clinic – if additional and more structured approach

The Service helps people in Powys (aged 18+) and their families or carers, to access community-level services and activities that will help them maintain independent lives and which help prevent their circumstances deteriorating to a point where they might need higher level health or social care services.
