Registering with the surgery
We ask you take your medical card to any of our three surgeries or you can complete the GMS1w (PDF) registration form and either the adult health questionnaire (PDF) or the child health questionnaire (PDF) form in order to register with us.
Following registration with the practice all patients over the age of five years can request, or have requested on their behalf, a registration clinic for a routine health check.
We believe strongly in preventative medicine and our clinical staff are pleased to give advice on blood pressure checks, cervical smears and immunisations.
To register you will need to live within our catchment area.
When you register, it’s helpful to have your NHS number. Your NHS number is a 10 digit number that you find on any letter the NHS has sent you. For example, 485 777 3456.
Patient preference
We will record in writing any preference expressed by the patient (or, in the case of a child or an adult who lacks capacity, the person who made the application on the applicant’s behalf) to receive services from a particular performer, either generally or in relation to a specific condition.
